Izuku Midoriya and Tomura Shigaraki (緑谷出久と死柄木弔 Midoriya Izuku to Shigaraki Tomura?) is the one hundred and thirty-first episode of the My Hero Academia anime and the eighteenth episode of the sixth season.
Summary[]

Izuku meets the former One For All heirs.
While unconscious in the hospital from the aftermath of the Paranormal Liberation War, Izuku once again finds himself inside the subconscious realm of One For All. However now, all off the vestiges are fully formed and able to properly communicate with him, which came as a result of One For All's rapid growth and Tomura dragging them out in his attempt to steal the Quirk using All For One. The vestiges wish to have a critical discussion with Izuku, whose vestige is still partially obscured, before he gets the chance to wake up in the real world.
The fourth user, Hikage Shinomori, first explains to Izuku that on top of Nana's Float during the boy's fight with Tomura, he manifested his Quirk: Danger Sense, which allows a user to suddenly detect any incoming harm. Izuku recognizes the name from the notes All Might tried to take on the previous users. Hikage's section mentioned he died at forty years old, but the cause of death wasn't fully written and was quickly crossed out. Hikage explains that All Might's research uncovered that he ended up dying of old age.

Hikage explains the drawbacks of One For All.
After determining that he wasn't strong enough to take down All For One at his current level, he spent the last eighteen years of his life stockpiling as much raw strength into One For All before passing it on. During his final years, he ended up gaining two mysterious cracks on his face, which he originally thought was from an unknown disease or him not training hard enough.
According to Hikage's autopsy, the cracks and the shortened lifespan were caused by Hikage's body being unable to handle multiple Quirks at once. Izuku questions his deduction, as All Might had the Quirk for much longer than the fourth user at forty years. The First User then elaborates that it's because All Might was Quirkless, so his body was able to naturally accept the Quirk being entered into it due not having one to begin with. This also allowed him to fully draw out the power of One For All without any drawbacks, as it became his "given" Quirk by default.

Izuku may be the last person able to wield One For All.
This revelation inadvertently creates another problem for the users: due to the vestiges Quirks appearing, and how dangerous the Quirk is for someone who was already born with one, it's unlikely they'll be able to pass it to another person unless they are also Quirkless. With fewer and fewer Quirkless people being born with the passage of time, Daigoro surmises that Izuku would likely be the last person to carry One For All.
With all the groundwork laid down, Nana then tells Izuku the main issue of their sudden meeting: whether or not he has the resolve to kill Tomura if the situation comes to it. While Nana and the others acknowledge what Izuku felt in his last interaction with Tomura, the spirit of Tenko asking to be saved, they place equal importance on how little that might mean due to the intense hatred instilled into him from being raised by All For One. Enough to where the users surmise it came to be from another longstanding plan to steal One For All.

Nana asks Izuku if he has enough resolve to kill Tomura.
All For One can potentially steal it using intense negative emotions to override the other Quirk's anti-theft properties. All For One previously tried to steal One For All from Daigoro and En, the sixth user, but failed, so it's their belief the man is using Tomura to funnel and amplify his hatred to take the Quirk by proxy.
Nana once again asks that even if he could connect with a small part of Tomura, and under the growing threat of him achieving his goals and becoming an unstoppable, irredeemable monster, if Izuku would still have the resolve to stop him regardless of whether or not killing Tomura was on the table.

Even though it may be too late, Izuku wants to save Tomura.
Izuku tells Nana that, compared to other Villains he's fought, Tomura is the only one who he understands why they turned to evil. Unlike his other opponents who he fought with the sole purpose of stopping them, Izuku could feel the spirit of Tenko crying out for help underneath all of Tomura's hatred which actively complicates.
While he understands that fighting still might be an option, and Tomura has done many horrible things, he believes One For All isn't a power meant for killing in the present day. He believes it's now become a power meant save, which he learned from All Might and how he and the other users' usage of the Quirk has inspired people over the years. Izuku, now fully formed in the vestige world, states he wants to save the little boy inside Tomura, even if he's not completely sure about the prospect of killing him if things come to that.

Toshinori reveals the truth of One For All.
The First User agrees with Izuku's answer and proclaims that he and everyone else will follow him and says he's proud that him and All Might were able to receive the Quirk. Nana then apologizes to Izuku for testing him like that, also appreciative of his answer, and asks him to say hi to Gran Torino for her when he wakes up. The First User then tells the Second and Third Users, who purposely excluded themselves from the conversation, that they should start helping out to unleash the full power of One For All.
Back in the real world, Hawks and Best Jeanist arrive in Izuku's room to speak with All Might about One For All. After Hawks elaborates that this information has spread to the public and could breed more unrest in regard to their trust in Heroes, All Might agrees to tell the two everything in private.

"Just watch me!"
Three days later, the press conference featuring Best Jeanist, Endeavor, and Hawks is held. Endeavor verifies Dabi's claims and comes clean about his family history to the public. Hawks also publicly reveals the fabricated murder of Best Jeanist to refute one of the claims, though confirms his statements about his own family history and Twice's murder. He then says that while he takes responsibility for not getting through to Twice and apprehending him in time, his killing was a necessary last resort due to the unfathomable damage Double would cause if it went out of control.
The three then reveal that in order to properly deal with the lack of Heroes and the exploding population of Villains, all the citizens will start being evacuated to U.A. High School and other hero schools for their safety. The reporters become even more upset and believe their calm demeanor isn't a proper solution to all their issues. Endeavor states that while uncomfortable, it's a necessary step to make sure they work towards undoing all the problems set off by the War, and anyone's grievances towards the other Heroes should be piled on to him.

Izuku leaves a farewell letter to his classmates.
At the start of April, Class 1-A each receive a message of their own from Izuku in the form of a letter and his abrupt absence. In it, Izuku reveals the secret about One For All, his targeting by All For One and Tomura, and that he left U.A. in order to keep them safe. The letter ends with Izuku thanking his classmates for the time they spent together and tells them goodbye.
Elsewhere in the city, Izuku, now wearing his damaged hero costume, looks off from a building at a sudden explosion in the distance. He emotionlessly remarks how huge the supposed Villain is, puts on his tattered mask, and moves out.
